That file mysteriously disappeared in 3.1.22. It was included in 3.1.21,
and a copy of the file is available as
ftp://ftp.edu.stockholm.se/pub/siag/makeicons.
It will be back in 3.2.0, whenever that is released. A prerelease should
be ready soon.
